Agreement for Team Participation
The Agreement for Team Participation (Agreement) is a waiver of liability for student participation in a “Team”. Examples of a Team include the following; any sport; Cheerleading; Dance; or Marching Band. The main criteria in determining if an activity constitutes a Team is if the activity is extra-curricular, has voluntary participation. Such activities typically include training and receiving coaching, and involves Team events such as competitions, shows, and performances.
The Agreement is to be signed by each student and their parent/guardian prior to commencement of the Team’s activities. A separate Agreement is to be completed for participation in each Team. For example, if a student competes in football and basketball, the student and parent/guardian need to complete an Agreement for each sport.
This Agreement must be completed and on file with the school prior to the student’s participation in the activity in any manner.

Alternate Transportation
Students participating in off-campus district-sponsored activities, including, but not limited to, practices, games, meetings, competitions, and conferences (“Events”), may travel on school buses or by other district-designated methods of transportation. Under certain circumstances, and with the district’s prior written approval, students may be transported to and from events (a) by a parent/guardian or other designated adult, or (2) by himself/herself. Under no circumstances may students be transported in a vehicle driven by another student or anyone under 21 years of age.

Concussion and Head Injury Report Forms
AB25 was signed into law this year, and will take effect January 1, 2012. A copy of the law is here. AB25 pertains to suspected or potential concussions and head injuries (CHI) from student participation in athletic and related extra-curricular programs. The Risk Management Department has been working in conjunction with our Athletic Director and Schools Insurance Authority (SIA) to develop a compliance program.
Ed Code Section 49475, requires students and parents be provided with a “concussion and head injury information sheet” that must be signed by all students/parents involved in covered activities occurring on and after January 1, 2012. The statute does not state what specific information must be legally provided. To ensure district compliance, the provided form contains information regarding the law, how the district will implement the law during athletic events, and steps that should/must be taken by parents in cases of potential concussions and/or head injuries.

Heat Illness Information Sheet
On June 1, 2018, AB 2800 – California High School Coaching Education and Training Program: heat illness, was signed into law by Governor Brown that requires coaches when renewing their CPR/FIRST AID, Concussion and Sudden Cardiac Arrest (SCA) certification that they also complete separate training in the signs and symptoms of heat illness.
A FREE online course “Heat Illness Prevention” is available through the CIF and NFHS, and will satisfy this mandated requirement by California law.
To help educate parents and student/athletes about Heat Illness warning signs and risk factors, an information sheet will be signed annually by the parent, guardian or caregiver and their student/athletes.
Sports Physical Exam
The purpose of the Sports Physical Exam form is to ensure that students who participate in extra-curricular activities that are physically demanding are capable of meeting those demands. It is imperative that this form be completed by a Medical Doctor (M.D.), a Doctor of Osteopathy (D.O.), a Nurse Practitioner (N.P.) or a Physician’s Assistant (P.A.). Physical exams conducted by a chiropractor, physical therapist or any other medical professional will not be accepted.
The completion of the Sports Physical Exam Form is good for one year, even if the student participates in multiple sports or activities during the course of the year. The student should indicate on the form which sporting events/activities they will be participating in during the course of the year.

Sudden Cardiac Arrest Information Sheet
On January 30,2015, the California Interscholastic Federation Federated Council voted unanimously to amend CIF Bylaws 22.B.9 and 503 to include language that adds Sudden Cardiac Arrest training to coach certification and practice and game protocol that empowers coaches to remove a student/athlete who exhibits fainting – the number one warning sign of a potential heart condition.
A student/athlete who has been removed from play after displaying signs or symptoms associated with SCA may not return to play until he or she is evaluated and cleared by a licensed health care provider.
To help educate parents and student/athletes about SCA warning signs and risk factors, an information sheet will be signed annually by the parent, guardian or caregiver and their student/athletes.
