ELIGIBILITY REQUIREMENTS TO PARTICIPATE
EGUSD students who:
- have no health insurance or insurance that does not cover vaccines, or
- are Medi-Cal eligible, or
- are American Indian or Alaskan Native
WHAT YOU NEED TO BRING
- A parent or legal guardian must accompany child.
- Bring your child’s Immunization Card (yellow card) or “Out of Compliance” letter from the school.
IMMUNIZATIONS PROVIDED
- All immunizations required for school entry will be provided for students Preschool through 12th grade wanting to enter into Elk Grove Unified School District.
LOCATIONS: Maeola Beitzel Elementary, 8140 Caymus Dr, Sacramento, CA 95829
Clinics will be held in the Multi-Purpose room. Families can go straight to the Multi-Purpose room for their appointment.
